Eyelid surgery · Under-eye bags

Correcting under-eye bags.

When the elasticity of the lower lid tissue slowly declines with age, the fat from the eye socket often bulges forward and excess skin forms in the lower lid region: the so-called under-eye bags. They can be corrected on an outpatient basis at the Kuzbari Centre in the course of a low-risk lower eyelid lift.

Why under-eye bags develop

The lower lid tissue loses elasticity over time.

Over the course of a lifetime the elasticity of the lower lid tissue gradually declines. The lid skin, the muscles and the connective tissue beneath them slacken. The fat of the eye socket bulges forward increasingly. These changes in the tissue lead to the typical signs of ageing around the eyes.

The so-called under-eye bags arise either from an excess of lower lid skin or from the bulging of orbital fatty tissue as a result of age-related slackening of the tissue. Frequently there is a combination of both. Incidentally, under-eye bags contain no tears but fatty tissue. The actual tear production takes place in bony hollows at the side of the nose.

Removing under-eye bags: lower eyelid surgery at Kuzbari

Natural results that preserve your facial expression.

In the field of eyelid surgery the doctors of the Kuzbari Centre are among Europe's top specialists. They are regularly invited to international congresses to speak on the subject.

When under-eye bags are corrected, the goal must always be a natural result and the preservation of the individual's facial expression. At the Kuzbari Centre we specialise in eyelid surgery and help patients back to a fresh appearance.

In lower eyelid surgery the excess of skin and fatty tissue is removed and the under-eye bags are thereby eliminated. Patients can go home again shortly after the outpatient operation — and soon look fresh and alert once more. That an operation has taken place usually goes unnoticed by friends and acquaintances.

What the eyelids give away

The eyes give away your age — and influence the people around you.

The eyes give away your age. Along with the hands, the eyelids often give away our true age, because the natural ageing process starts considerably earlier and faster in the thin layers of skin on the lids than anywhere else.

You influence the people around you. Hooded eyelids and under-eye bags have effects — on those affected as much as on the people around them. The way we perceive unfamiliar faces is the way we approach the people behind them: grumpy, listless, unfriendly … And because we approach people with these negative feelings, they react to it, and the next vicious circle begins.

A patient's story

Tired eyes before — a fresh gaze now.

Using one patient as an example, DDr. Jessica Wittmann shows how an eyelid lift at the Kuzbari Centre makes a tired gaze look fresh and alert again — naturally and without changing the face.

Virtually scar-free

Precise access routes — barely visible scars.

In some cases the lower eyelid correction can be reached through an access route on the inside of the lid. No skin incision at all is then needed. In other cases a precisely placed incision is made just below the lashes of the lower lid and — depending on the extent of the tissue slackening — a few millimetres beyond the outer corner of the lid as well.

After some months the scars are often no longer visible at all, or only detectable on very close inspection.

In conversation with Dr Grill

Naturalness through expertise

Many people have deep and well-founded reservations about facial surgery, because there are repeated examples of unnatural, mask-like faces after an eyelid lift. In this video Dr Christoph Grill explains what matters in eyelid surgery, what risks and dangers there are and how to arrive at a natural result.

How it works

The path to a radiant gaze.

  • 1. Consultation

Everything begins with a consultation lasting about an hour. You get to know us and already receive important information about the possible treatment options. You also find out what the treatment costs and how long it takes.

  • 2. The procedure

The lower eyelid lift is usually carried out on an outpatient basis under twilight sedation. The under-eye bags are removed by repositioning slackened skin, muscle tissue and protruding fatty tissue. In addition, the body's own fatty tissue is grafted at the transition between the lower lid and the cheek.

  • 3. The result

The waking-up phase takes place in our state-of-the-art recovery room under the supervision of our trained staff. You should not drive yourself home after the procedure. Your lids are heavily swollen at first — but that changes quickly.

The course of healing

The first days and weeks after the correction of under-eye bags.

The first days. The swelling still increases over the first 3 days. Cooling the eyes eases the swelling. Physical exertion should be avoided. You can go to work, but you have to expect to look operated on for the first 2–3 weeks. With lower eyelid procedures in particular, irritation of the conjunctiva is common — anti-inflammatory drops and ointments bring the problem under control.

The first weeks. The appearance of your eyes will normalise after about 3 weeks. Your whole face now looks fresher, without looking operated on. A slight residual swelling only subsides after 2–3 months. A final assessment of the result takes place after 6 months.

Frequently asked questions

Answers about under-eye bags

What causes under-eye bags?

Under-eye bags arise from fatty tissue that accumulates under the eyes, often through ageing or genetic predisposition. We remove it precisely for a fresh appearance.

How does the operation to remove under-eye bags work?

The operation is usually carried out through a small incision on the inside of the lid, so that no visible scars arise. The procedure takes about 60–90 minutes.

How quickly will I see results after the removal of under-eye bags?

The first results are visible after 1–2 weeks. The final outcome shows after about 2–3 months, once the last residual swelling has subsided.

Are there risks in removing under-eye bags?

Risks such as swelling or temporary dryness of the eyes are small and are minimised by our experienced surgeons.

Can under-eye bags be treated without an operation?

Non-surgical methods such as fillers can disguise under-eye bags, but are often less effective. Our doctors will advise you on the best solution.

How long does the result of under-eye bag removal last?

The result is as a rule permanent, since the fatty tissue that has been removed does not come back. Our techniques ensure long-lasting results.

Are under-eye bags a medical problem?

As a rule no — they are an aesthetic matter. The bulging fatty tissue is harmless, but it makes the face look permanently tired and exhausted, no matter how well rested you are.

Why do creams not help against under-eye bags?

Because under-eye bags are not a skin problem but bulging fatty tissue behind a slackened supporting structure. No cream in the world shifts fatty tissue. Cooling products can briefly ease morning swelling — but they change nothing about the underlying condition.

What is the difference between under-eye bags and dark circles?

Under-eye bags are a bulge (too much tissue), dark circles a hollow or shadow (too little volume, or vessels showing through). The two can occur together. We treat under-eye bags surgically and dark circles often with hyaluronic acid — at the first appointment we establish which applies to you.

Am I awake during the operation?

The procedure is carried out under local anaesthetic, with twilight sedation on request. You feel nothing during the operation.

Does health insurance pay for the removal of under-eye bags?

We are a private centre — coverage by statutory health insurance is not provided for with us. The differences between the two routes are compared on our page Private or health insurance?

Who carries out the operation?

Exclusively our specialists focused on eyelid surgery: Dr Christoph Grill, DDr. Jessica Wittmann or Dr Shawqi Arafat. Whoever advises you also operates on you and looks after you until the final result.
